NAD+: Compounded Cellular-Energy Support Under Physician Supervision

The Pep Club Medical Team3 min readUpdated May 12, 2026

What is NAD+?

NAD+ (nicotinamide adenine dinucleotide) is a small molecule present in every cell. Cells use NAD+ as a coenzyme in many of the enzymatic reactions involved in energy metabolism. The molecule is consumed continually and is resynthesized by the body from dietary precursors.

The NAD+ preparations available through The Pep Club are compounded by a licensed compounding pharmacy and prescribed under U.S.-licensed physician supervision. Each prescription is issued after a clinical intake and consultation.

Who can be considered

Patients evaluated for NAD+ commonly describe persistent fatigue or are following an integrative wellness plan reviewed by their physician. Eligibility is determined at consultation. Active cancer, pregnancy, breastfeeding, severe liver disease, and concurrent chemotherapy or immunosuppressant use are contraindications. The physician reviews all current medications and prior reactions before issuing a prescription.

Safety and follow-up

Safety note

Patients sometimes report transient chest tightness or pressure during IV infusion (rate-dependent and resolves on slowing the infusion), mild nausea, flushing or warmth at the injection site, headache, or short-lived fatigue at the start of therapy. Patients are instructed to contact the practice if any unexpected reaction occurs. Follow-up consultations occur at the cadence determined by the prescribing physician.
